1. BUSINESS QUESTION:
How have heart failure admissions and mortality outcomes changed over time, and which regions and age groups should be prioritised to reduce healthcare pressure and mortality?

2. KEY INSIGHTS:
- Hospital admissions increased by 26.2% over the past five years, highlighting growing pressure on cardiovascular services.
- There were 123K deaths within one year of admission in 2022, an increase of 19.1% in the past 5 years.
- Despite rising absolute numbers, the overall mortality rate has gradually declined, indicating improvements in care and survival outcomes.
- The Midlands recorded the highest admissions, indicating a disproportionate regional burden and potential capacity strain on cardiovascular services.
- Individuals aged 75+ account for nearly 67% of all admissions, making them the most affected demographic group.

3. RECOMMENDATIONS
- Allocate additional resources to the cardiovascular services in the Midlands, to address the highest admission volumes and regional demand imbalance.
- Prioritise the Midlands for preventative programmes for heart failure to reduce avoidable hospital admissions.
- Expand digital or community based monitoring and post-discharge support programmes for patients age 75+ to improve outcomes and reduce readmissions. Targeting this demographic offers the highest potential impact, as they account for nearly 67% of admissions.
